Who We Are:

The Community Living Program is a division of Southwest Community Services of Catholic Community Services, Inc.  The CLP has provided quality and individualized residential support services to individuals with developmental disabilities since the 1980’s. The CLP is a Qualified Vendor of Program Services for the Arizona Division of Developmental Disabilities.

Who We Serve:

The CLP is proud to support individuals who need some assistance in accessing their home, their community and resources in order to live personally satisfying lives in their own homes.  Typically, we provide service to adults with multiple disabilities that may include cerebral palsy, mental retardation, autism and/or epilepsy, as well as sensory deficits, and secondary mental health diagnoses. Most people are referred to us by DES/DDD, which determines each individual's eligibility for services.  Each person’s individual program team determines the level of care and/or supervision needed for each individual to be successful and safe in their own home.   Supports are put in place depending upon each person.
