Types of Residential Programs offered by the CLP:

Group homes:
The CLP  group homes allow adults with developmental disabilities to live in a home setting while receiving 24-hour care and support from licensed providers. Integrated into regular neighborhoods,  these homes accommodate four to five individuals.  The homes have been modified with wheel-chair ramps, roll-in showers, and wider doorways to make them move accessible to residents.   All homes maintain the required programmatic and health and safety standards for licensure.

Apartment Living:
The CLP maintains apartments in various community apartments complexes for individuals who wish to live on their own or with roommates.  The CLP staff provide support to individuals for tasks of daily loving such as bathing. dressing, cooking, cleaning, shopping, as well as medical appointments and therapy.   Staff also assist residents to set up, attend, and participate in their choice of community activities, recreational outings and use of regular community resources.  All apartment settings are individually licensed with the State of Arizona and follow the standard program monitoring rules and regulations. 

In-Home Service
Some individuals prefer to live in their own homes or with their families. Like those in the CLP licensed apartments, they need assistance with daily tasks. Based on a plan designed for each individual, CLP staff members visit the person's home each day and provide the needed assistance.

Health Care and Behavior Plan Management:

CLP staff members assist in managing the health care of residents in our licensed group homes and apartments. They take residents to appointments, order and administer medications, report medical needs to families and case managers, and carry out any other necessary tasks assigned by health care professionals.   In addition, CLP staff members develop, implement, and monitor behavior management plans for some individuals who need assistance in working through behavioral challenges. All behavior plans are rooted in positive techniques and provide a means for all individuals to express their wants, needs and desires in a way that works effectively for each person.

The Community Living Program - Making a Difference!

The CLP has implemented various management and training techniques that keep Administrators, Staff, Colleagues and Funding agents current with information and communication regarding all aspects of the program operations.  Staff members use www.Therapservices.net to document all data regarding individual programs and activities.  Administrators are able to track up to the minute notes and progress input by staff and can access that information from any computer in the world that has web capacity.  Staff members can also access web-based training programs to enhance skills and understanding of providing services to people with developmental disabilities.
